Wraparound services are an ecologically based process and approach to care planning that builds on the collective action of a committed group of family, friends, community, and professionals. It's a
cross- system support that mobilizes resources and talents from a variety of sources resulting in the creation of a plan of care that is the best fit between the family vision and story, team mission, strengths, needs, and strategies. Wraparound is a planning process that follows a series of steps to help children and their families realize their hopes and dreams. This process helps make sure
children and youth grow up in their homes and communities. It is a planning process that brings people together from different parts of the whole family’s life. We keep the caseload to ten consumers per case manager on purpose. There is so much work that goes in to helping these families help their children and youth maintain stability and regain the best possible level of functioning.
ELIGIBILITY:
Consumers must be under the age of 18, diagnosed with a clinically significant disorder of thought, mood, perception, orientation, memory, or behavior. Demonstrate clear evidence of functional impairment in at least two of the four domains and have persisted for at least one year or is judged by a behavioral health professional to be at high risk of continuing for one year without professional intervention. Must have a CASII or ESCII score of four or higher. High-Fidelity Wraparound is represented in many unique ways. We LOVE our turtle mascot, as it has encouraged many families to remember we all travel at a different pace. Through this program we pride ourselves in meeting families exactly where they are.
