Care coordination is an activity rather than a
service. Care coordination is deliberately organizing consumer care activities and sharing information among all of the participants concerned with a consumer’s care to achieve safer, more appropriate, effective care. This means consumer’s needs and preferences are known ahead of time and communicated at the right time to the right people.
Care coordination is person-centered and family-centered to provide an integrated and coordinated approach
to addressing all aspects of an individual’s health. Person-centered and family-centered care considers the consumer’s
choice in services provided, as well as the physical, behavioral health, and social service needs of each individual as these factors influence the well-being of the whole person.
ELIGIBILITY CRITERIA:
Every Pathways consumer is entitled to Care Coordination
services.
