Pathways provides full access medication management services to consumers of all ages, when clinically indicated. Pathways’ medication management provides support, in addition to behavioral interventions to consumers struggling with an array of mental health and substance use issues. In addition to regularly meeting with consumers to monitor response to medication, Pathways prescribers will monitor various lab results to ensure medication safety and compliance.
ELIGIBILITY:
Consumers under the age of six must be staffed with a local prescriber prior to scheduling appointment. If a consumer is inappropriate for medication management at Pathways, a referral to a specialist will be provided.
TO MAKE A REFERRAL:
To make a referral for medication management contact, Pathways Scheduling Center. In the event that a consumer has an immediate need and there is no standard prescriber appointment available, contact the local Customer Service Manager/ Customer Service Representative to utilize Medical on Demand where a nurse will evaluate for the need of an emergent appointment.
