Participant Directed Services (PDS) provides the participant and family with an opportunity to design and direct their self-identified community support needs. Services are available in all ten counties served by Pathways.
PDS participants must:
• Work with assigned support brokers/service advisors to develop individual service plans that include support spending and emergency backup plans.
• Hire, train, schedule, and terminate employees as necessary.
• Follow the person-centered service plan (plan of care) and spending plan.
• Meet any patient liability obligations as determined by the Department for Community Based Services.
• Choose an authorized representative, if needed or desired.
• Submit paperwork correctly and timely.
• Follow PDS policies, procedures, guidelines, and administrative regulations.
ELIGIBILITY:
An individual must have an intellectual disability prior to the age of 18. They must have at least two deficits in adaptive functioning areas, and/or have a physical disability prior to the age of 22, or three or more deficits or impairments in the areas of self-care, learning, mobility, self-direction, use and understanding of language, or capacity to live independently.
